    About three years have passed since then, and this field has changed dramatically. Some of the overseas services I originally featured have shut down their apps, others have been absorbed into larger companies, and some have undergone major pivots.

    Behind these shifts is the rapid proliferation of large language models (LLMs) like ChatGPT and Claude. We've entered an era where people can simply consult general-purpose AI instead of downloading dedicated apps — and this has fundamentally reshaped the industry.

    In this article, I'll provide a comprehensive overview of the latest global and Japanese services as of February 2026 and share my thoughts on where the market is heading.

    A Dramatically Changed Landscape Since 2023

    Let me start by reviewing what has happened over the past few years.

    The Rise of LLMs and Their Impact on the Industry

    Since 2023, general-purpose LLMs like ChatGPT and Claude have exploded in popularity, profoundly impacting the AI counseling industry.

    • Approximately 49% of LLM users with self-reported mental health issues already use LLMs for mental health support
    • In March 2025, the Dartmouth "Therabot" trial was published in NEJM AI — the first RCT (randomized controlled trial) of a generative AI therapy chatbot, reporting a clinically significant 51% reduction in depression symptoms and 31% reduction in anxiety symptoms
    • The reality that "you can consult an AI you already have without downloading a dedicated app" has challenged the very value proposition of specialized apps

    Evolving Regulatory Landscape

    As technology advances, regulations worldwide are catching up.

    United States: The FDA convened its Digital Health Advisory Committee in November 2025 to examine regulatory frameworks for generative AI-enabled mental health devices. Illinois enacted a law (effective August 2025) prohibiting AI from making independent therapeutic decisions.

    EU: The AI Act took effect in mid-2024, with full implementation rolling out through 2026. Mental health AI tools will face tiered requirements based on risk level.

    Japan: The "AI Promotion Act" was enacted, positioning AI as "the foundation of economic and social development" while mandating transparency to protect citizens' rights. In April 2025, the Japan AI Mental Health Care Association published draft "Guidelines for Using Generative AI in Mental Health Care Businesses."

    Market Size

    The global AI mental health market was valued at approximately $1.7–1.8 billion in 2025, projected to reach $11.8–25.1 billion by 2034, with a compound annual growth rate of 23–32%.

    The Current State of Overseas Services

    What Happened to the Pioneers

    Let me be straightforward about what happened to the three services I featured in 2023.

    Woebot: End of a Pioneer

    Woebot, which pioneered the AI counseling app space with its CBT-based approach, had achieved FDA Breakthrough Device Designation in 2021 and raised a cumulative $123.5 million in funding.

    However, the consumer app was shut down on June 30, 2025. Founder Alison Darcy stated that "AI is moving faster than regulators." The inability of the FDA to provide clear regulatory frameworks for LLM-based tools proved to be a major obstacle.

    The company has since pivoted to a B2B model, serving only through healthcare providers and insurers.

    Ginger: Merged into Headspace

    Ginger, which provided mental health support combining AI with human professionals, merged with Headspace in 2021. The resulting "Headspace Health" was valued at $3 billion.

    Today, ginger.com redirects to Headspace's website. Ginger's AI coaching capabilities have been integrated into Headspace's platform, evolving into a comprehensive mental health service combining meditation, mindfulness, and clinical support.

    Replika: Pivot to Wellness

    Replika, formerly popular as a personalized AI chatbot, has undergone a significant transformation.

    In 2025, Italy's data protection authority fined the company €5 million for GDPR violations, leading to significantly strengthened safety filters. Replika has clearly repositioned from a romance-oriented app to a wellness companion, now emphasizing mental health and self-care.

    Currently Notable Services

    As pioneers pivoted, new services have risen to prominence.

    Wysa: Leading Clinical Validation in AI Mental Health

    India-based Wysa is arguably the most clinically validated AI mental health app currently operating at scale.

    • Over 7 million users across 105+ countries
    • Evidence-based approaches including CBT, ACT, and DBT
    • 150+ therapeutic exercises
    • Partnerships with the UK's NHS, Singapore's Ministry of Health, and other public institutions

    With the successes and failures of overseas services in mind, let's turn to the Japanese market. Due to language and cultural barriers, overseas services can't simply be imported as-is, and Japan has been developing its own unique services.

    Awarefy

    With over 700,000 cumulative downloads, Awarefy is Japan's most well-known AI mental health app.

    • 24/7 chat support with AI mental partner "Fy"
    • Built on cognitive behavioral therapy (CBT) and mindfulness
    • Mood/emotion tracking and visualization with weekly reports
    • 200+ audio guides (meditation, sleep, nature sounds)

While pioneers like Woebot and Ginger have disappeared, new services like Wysa and Talkspace are forging new paths with hybrid models and proprietary LLMs. In Japan, services like Awarefy, emol, and SELF MIND are evolving in their own unique ways.

    However, the industry as a whole still seems to remain within the framework of "AI as a tool for coping with symptoms."

    What we want to ask goes one step further: Can AI become a "companion" that helps people understand themselves more deeply and grow? RefCla is our answer to that question.
